DSA provides advice and recommendations where behaviours are impacting the care of a person living with dementia. This advice is tailored to each individual and the environment where they are receiving care.
In order for DSA to provide appropriate strategies and recommendations we require information from the care provider. DSA has developed a range of Assessment Tools that are available to download and use to gather information on the person with dementia and the behaviours.
Lifestyle and Social History Questionnaire
Knowing a person's life story helps DSA understand the person, what is important to them, their likes and dislikes. This information enables DSA to provide advice and recommendations that are unique and tailored to the individual.
