Assessment Tools

DSA provides advice and recommendations where behaviours are impacting the care of a person living with dementia. This advice is tailored to each individual and the environment where they are receiving care.

In order for DSA to provide appropriate strategies and recommendations we require information from the care provider. DSA has developed a range of Asssessment Tools that are available to download and use to gather information on the person with dementia and the behaviours.

Lifestyle and Social History Questionnaire

Knowing a person's life story helps DSA understand the person, what is important to them, their likes and dislikes. This information enalbes DSA to provide advice and recommendations that are unique and tailored to the individual.

This form is to be completed when a significant incident takes place.

Abbey Pain Scale

People with dementia may not be able to verbally communicate that they are in pain. This tool assists in measuring pain in people who cannot verbalise.

Sleep Observation Form

Dementia can affect a person’s sleep patterns for a variety of reasons and are a common occurrence.  It is important to recognise what may be causing this as it will then help decide on which strategy may be helpful.

This form helps monitor a person's sleep cycle and document any strategies attempted.
